HOW TO APPLY
To become a candidate for assistance, please download the application form and contact your surgeon, oncologist, GP or psychologist to request they nominate you for funding.

Research the provider you are considering using for your tattoos.

Consult with the provider, to discuss:
a) your breast cancer procedure and surgeries;
b) your scars, current medical condition, medications, etc;
c) your tattoo designs; and
d) your intention to apply for funding and the payment terms of the funding. -
Obtain a written quote from the provider and a copy of their standard price list (if available).

For camouflage/artistic tattoos, obtain the Master Licence numbers of tattoo studios and the Licence Number of the tattoo artist.

Complete, initial all pages and sign this Application Form and ensure all requested documentation is attached.

Meet with your treating medical professional for their review of the completed Application and attachments and obtain their signature.
Once you have completed all stages of the application process, your nomination will be considered by Pink Ink (Australia), and the Board will notify the applicant if they have been selected as a candidate.
If approved, and once you have accepted the grant, Pink Ink (Australia) is released from all liability for the said grant and procedure.
The form is a fillable PDF with signature fields. All fields are mandatory.
